Application Checklist 5785

This is a list of everything you need to complete with your application:

1. Online application form including:
· Personal information
· Questionnaire
· Medical form
· Tznius form
2. 9-12th grade Transcripts
3. 11th and 12th grade Report Cards
4. Family form
5. Medical form filled out by Dr
6. Mechaneches form
7. 2 reference forms

Once your application is complete, an interview will be scheduled closer to the time, and you will be notified through your school. Interviews will take place between: Motzei Shabbos 24/02 - 'ט"ו אדר א and Wednesday 28/02 - 'י"ט אדר א.

The application fee is $150- to be paid in cash only at the interview

The test will take place on Wednesday afternoon 28/02 - 'י"ט אדר א. We will confirm the time and location closer to the test date. The test is mandatory and other interviews cannot be made during this time.
